Who pays for it?

As the healthcare debate winds down in DC, we find we still haven't
answered the big questions that make it so hard to come up with a
political solution: are we responsible for our own health? Is there a
right to care? How much of are wealth should we invest in extending
life? Are some people more worthy of the best care than others?

Until we at least admit that these are the questions, our answers will
continue to be incomplete at best.
